For that to be real, the underlying planchet would have had to stuck to the die and then this planchet was struck next, rotated and partially ejected and then restruck with the underlying piece remaining stuck to the die. For the brockage image to be so sharp, it must have been the first and second strike. If it wasn't done on purpose, then it is a fantastic error piece.
