mmarotta
No one replied, so I will hazard this: It is one of a series of medallions struck by the Paris Mint, each to commemorate a famous French building. La Conciergerie was a palace and prison. http://conciergerie.monuments-nationaux.fr I cannot read all of the obverse at 9 o'clock to 6, but it seems to say "... monuments historique..." and that is plain to understand.
