What is it?

It is some sort of commemorative medal for the Makarovsky Zheltovodsky monastery in Sarans'k Mordovia.

As additional information, the email I got said the following:

"This is a bronze coin, 30 mm. diameter.It depicts the Makaryev Monastery on one side, and St. George on the second one, denomination one denga. The date is 1620 (probably when the monastery was founded), but my guess is about 250 years older"

 

Having a Denga denomination didn't make much sense to me, hence the 'fantasy' speculation.

I think, it is a private souvenir jeton, definitely issued after 1991

and it's price is close to nothing - maybe a dollar or two.
