The one that is listed in more references (below) is only 10mm. As you can see only the star in the legend and the size difference make them different medals. The 18mm version is only listed in the Catalogo delle Medaglie .V. Secoli XVIII-XIX (which I call Milan for the city whose collection it catalogs) and that authority refers back to the smaller version in its cross-reference.
