The indent occurs when 2 blanks are fed into the same collar. When this happens one blank will partly overlay the other. When the hammer die strikes, the upper blank is driven into the lower blank causing a depression which is shaped roughly like the upper blank, but the pressure of the strike will deform both blanks.
