Im pretty sure this is a Sicilian coin... but in my Krause I can only find the obverse... does anyone know anything about this coin?

It appears to be a Sicily and Naples Piastra of 120 Grana, dated 1791 C#67, this is a one year type coin that is listed but not pictured in my 1983 Krause. It describes the reverse as a seated god at left, volcano in the centre, and a standing goddess at right.
