it is a weird one... but I do believe it is a brockage...

I have never seen a full brockage with an underlaying image...

Looks handmade to me. If so, how did it ever get through PCGS?

Looks good to me. It's struck through a late stage die cap where the metal of the cap is getting thin enough for the design of the die to begin showing through.
