I'm gonna go with the 2009 $15 coin from the Royal Canadian Mint. I don't think there's been a circulating piece since 1952, though.

 

 

There have been many with his effigy on since he died released as past the fact commemoratives as part of runs of British monarchs, but the one I am talking about was an actual circulating item.

i cant possibley see anything past 1952

unless it was 1953 and the die arrived late

I remember reading that they went chape and didn't wanna have new plates created, so they kept the GVI design. And then there were the Jersey "Liberated 1945" coins with QEII's portrait on them that were issued as late as 1960 I think, but 1945 was the only date on the coin.

 

In Canada when the monarch changed, but they didn't have the new hubs to create dies from, as they came from the British Royal Mint, they just stamped a dot(1936) or a maple leaf(1948 Et Ind Imp dropped).
